We are not in-network with “marketplace” plans i.e. BCBS Network E, Alliant, Cigna Connect, Ambetter or traditional Medicare (A&B) and Medicaid. If you are covered by these plans, you will be self-pay with our office.

Self-pay rates:

  • New Patient Evaluation: $200

  • Follow up visit: $100

We are also happy to accept Health Savings Account (HSA) and Care Credit.
